How To Submit A Referral Online
Step 1: Access the Referral Portal
- Visit our referral page from our website
- Click on the associated office you wish to refer to
- Enter your office email and main phone number
- New users verify email (takes seconds)
- Existing Intiveo users simply log in with their existing information
Step 2: One-Time Office Setup (First Use Only)
- Enter your office information
- Add referring providers
- Choose preferred tooth notation (Universal, FDI, or Palmer)
- Create password and accept HIPAA-compliant terms
Step 3: Submit a New Referral
- Click “Refer a New Patient”
- Select the NHOMS location
- Enter patient information
- Select referring provider and requested provider
- Choose procedure
- Add case notes
- Select teeth on the digital tooth chart
- Upload imaging/files (optional)
File Guidelines:
- Up to 10 files
- 50MB max per file
- Accepts PDF, DICOM, JPG, PNG, TIFF, STL, and common imaging formats
Click submit - you’re done!
Step 4: Track Referral Status
Log back in to:
- View referral status updates
- See appointment progress
- Add or receive notes
- Upload or download files
- Download referral PDF
When you opt in via email, you’ll know when:
- Patient is contacted
- Appointment is scheduled
- Treatment is completed

Do I need to create an account to send a referral?
Yes. Creating a secure login allows you to track referral status, communicate with our team, and securely transfer patient information. Setup takes about 2 minutes.
Do I need separate logins for different NHOMS locations?
Each NHOMS location operates its own referral portal.
The first time you refer to a new location, you’ll complete a short registration process for that office. Once added, it will appear in your referral list moving forward.
Why do I need to enter my office phone number?
This helps prevent duplicate accounts and ensures all referrals are properly associated with your office. It also helps us contact the right office regarding the right patient.
